Adult Medical and Liability Release

I, the "participant", by signing this agreement and in consideration of my participation in Community Bible SportsLife Program, LifeGroups or Events agree to the following:

I fully release and agree to hold harmless and indemnify Community Bible Church, employees, volunteers or agents, and any other sponsors or vendors and their respective parent companies, against any liability, injuries, damages, risks, claims, cost and expenses, including, reasonable attorney's fees, arising out of participant's involvement with Community Bible Church activities including, without limitation, any personal injuries (or damages to property) or aggravation to pre-existing injuries which may occur as a result of participation, even though that liability may arise out of negligence or carelessness on the part of the persons named in the release.  Community Bible Church is not liable for any injuries that occur while at a SportsLife, LifeGroup or event or after the event has ended.

I give my permission for Community Bible Church to take said participant to a doctor or hospital and hereby authorize medical treatment, including but not limited to emergency surgery or medical treatment, and assume responsibility for all medical bills.

I understand that Community Bible Church will not be liable if participant fails to cooperate with the rules and regulations, and that any infraction of the rules and regulations may result in immediate termination of the participant attendance in SportsLife, LifeGroups or events.  All expenses or cost associated with the participant's removal from the program shall be the responsibility of the participant, including transportation back to the original point of departure.

By checking the agreement box and entering the date below, I assert that I am the person registering and I assert that I have read and understand this agreement.  This Agreement expresses the complete understanding of the parties. This Agreement is binding upon myself, my heirs and my assigns.

Adult Photo Release

For valuable consideration received, I grant to Community Bible Church (“Photographer”) and its legal representatives and assigns, the irrevocable and unrestricted right to use and publish my likeness, my photograph(s) or digital image(s) and to reproduce my name(or any fictional name), or any image in which I may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium; and to alter and composite the same without restriction and without my inspection and approval. I hereby release Photographer and its legal representatives and assigns from all claims and liability relating to said photographs.
